The Transplant Wellness Program (TWP) would not be possible without healthcare providers connecting us with potential participants would who benefit from the program. The following are resources that you, as a healthcare provider, can access to help connect patients with the TWP staff.
How to request consent to contact in a usual clinical setting
Consent to Contact by Healthcare Team
- Verbal consent to contact: Use the verbal consent to contact for research purposes (with telephone script)
- In person consent to contact: Use the consent to contact for research purposes (signed by patient)
